Hospice Services

Through aggressive pain and symptom management, Hospice of the Southwest strives to ensure a peaceful and comfortable end-of-life experience, physically, emotionally and spiritually. Patients and families are considered integral decision-makers on the Hospice of the Southwest team and we incorporate their wishes in planning our care strategy. Patient status determines what services we deliver and by whom, as well as how often we visit. Our staff is on call and available 24-hours a day, seven days a week. We provide appropriate supplies, medications and equipment in accordance with the requirements of our patient’s medical diagnosis.

Hospice of the Southwest provides:

  • Nursing Care – a registered nurse monitors patient symptoms and medications
  • Social Services – a social worker gives emotional support and information on community resources and referrals
  • Spiritual Support and Counseling – a chaplain offers spiritual support and guidance for patient and family, facilitating meaningful interpersonal communication and leading religious ceremonies or rituals
  • Home Health Aides – a home health aide administers personal care for the patient such as bathing, feeding and nail care as well as light housekeeping and meal preparation
  • Trained Volunteer Support – a caring volunteer attends to other patient and family needs, including shopping, babysitting and carpooling that also allows caregivers a much-needed respite

We also supply:

  • Hospice-related medications
  • Durable medical equipment and comfort supplies such as hospital beds, oxygen tanks, wheelchairs and walkers
  • On-call emergency visitation and assistance
  • Bereavement counseling for family members for one year following death of loved one

In addition, the Hospice of the Southwest team is available to coordinate other services secured by patient or family, including physical, occupational and speech therapies typically not covered by Medicare.
